The prinicpal life care planners, Nancy Bond, MEd, CCM, CLCP, and Lynne Trautwein, MSN, RN, CCM, CMAC, CNLCP are both certified in life care planning and in case management. They have collaborative work experience for nearly two decades in the fields of complex medical needs, catastrophic health concerns and disabilities. Life care plans reflect this unique working partnership, merging principles of community health and nursing as well as special education, developmental issues and rehabilitation. As staff members at The Coordinating Center, they have access to the full resources of this innovative, family and person centered, multidisciplinary care management organization. They have a hands on knowledge of day–to-day issues confronting people with catastrophic needs, as well as the community resources to support them.
